An event every week that begins at 6:00pm on Tuesday, repeating until May 29, 2018

Day 1: Making the Decision (May 22)

This session is offered by the Dementia Society. This program will provide an opportunity for dementia caregivers to discuss considering the difficult decision to move their loved one into long-term care.

Special focus on:

  • Identifying and coping with the emotions of both caregiver and person with dementia.

Day 2: Understanding and Accessing Home and Community Care (May 29)

This session is offered by the Home and Community Care at the Champlain Local Health Integration Network (LHIN) to provide information on care services that are available. Patient-care coordination teams help develop care plans focused on maintaining independence and dignity at home and in community. They also guide the application process when long-term care placement is required.

Special focus on:

  • How do I access Home and Community Care?
  • What are the steps to access Long-Term Care (LTC)?
