About This Program

This program is designed to support people living with dementia in their homes so they can continue to live safely in their familiar environment, for as long as possible. Free virtual assessment by qualified Dementia Society-trained occupational therapists includes:

  • General and dementia-specific considerations for accessibility, home design and navigation, risks for wandering, fall risk reduction, management of activities of daily living (ADLs), and equipment/technology needs.
  • A personalized implementation plan and coordination of services to support assessment recommendations.
  • Technology training and ongoing support to safely increase social stimulation and connections.
  • Access to ongoing education and dementia-appropriate social and recreational activities.

This program is available at no charge to residents of Ottawa and Renfrew County.

NOTE: This program does not duplicate existing geriatric or social service in-home assessment programs.
