About This Program

Familiarize yourself with the changes that a move to long term care represents. Learn how to apply and what is involved in making the transition. The session will end with a QnA session with our expert speaker. This program is delivered in partnership by Home and Community Care Ontario Health East, The Dementia Society, and a caregiver with lived experience.
