The three-part video series will help you familiarize yourself with the changes that a move to long-term care represents. Learn how to prepare for the move and strategies for adjusting to the transition. The education series will help you learn about the process of making the transition.
This program is delivered in partnership by Home and Community Care Ontario Health East, The Dementia Society, and a caregiver with lived experience.
