Advance Care Planning Workshop at The Dementia Society

About This Program

This session is designed to support individuals and caregivers in understanding and planning for future health care decisions. Participants will learn about advance care planning, including how to identify and communicate their values, wishes, and goals of care. The session will also provide an overview of health care consent in Ontario, how to confirm your Substitute Decision Maker, and practical strategies for having meaningful conversations with the right people at the right time.

This program aims to empower participants with the knowledge and confidence to make informed decisions and ensure their wishes are understood and respected.
